Phlebotomy

  • You may receive up to 5 kits that will contain:
    • International Air Transport Association (IATA)/Department of Transportation (DOT) approved shipper
    • Vacutainer tubes
    • Labels
    • Freezer/gel packs if applicable
    • Federal Express packaging and prepaid airway bills
    • Packing instructions
    • Testing requisitions (if applicable)

  • Verify that the correct kit is being used by confirming the donor name and donor identification number (DID) listed on the outside of the kit
  • Match the donor date of birth and DID provided on the testing requisition and labels
  • Draw the tubes associated with each kit, this may include multiple samples to be shipped to separate locations
  • Complete the tube label with the collection date, collection time and phlebotomist’s initials
    Important: For confidentiality purposes, do not include the donor’s name on any of the tubes
  • Ensure correct tubes are placed in correct shippers and packed according to assembly instructions
  • Arrange for shipment the same day samples are drawn

Important: Call Donor Center Representative Immediately if

  • Kit is not received
  • Sample is not drawn
  • FedEx is unable to ship the same day
